Mechanism of Action

Elesclomol (STA-4783; Synta Pharmaceuticals) is a copper-chelating ionophore that binds Cu²⁺ extracellularly and transports it across the plasma membrane into mitochondria. FDX1 (ferredoxin-1) reduces Cu²⁺ → Cu⁺; Cu⁺ directly attacks lipoylated TCA cycle proteins — principally DLAT and DLST — causing their toxic aggregation and proteotoxic cell death via the cuproptosis pathway (Tsvetkov et al. Science 2022, PMID 35588000). Mechanistic rationale for SDH-deficient tumors: SDH loss constitutively upregulates FASN expression in SDH-deficient GIST cell lines and patient tumors (Rodríguez-Flores et al. Cancer Res 2026, PMID 41520938). FASN supplies mitochondrial fatty acid synthesis (mtFAS) with octanoyl-ACP, the obligate lipoic acid precursor via LIPT1/LIPT2. Elevated octanoyl-ACP → elevated lipoic acid → elevated lipoylated DLAT/DLST. This constitutively elevated lipoylated-protein pool is mechanistically predicted to sensitize SDH-deficient cells to FDX1-mediated Cu⁺ attack, lowering the cuproptosis threshold relative to SDH-intact cells. Clinical history: In Phase 3 NCT00088010 (n=651, advanced melanoma; elesclomol + paclitaxel vs. paclitaxel), the trial was stopped early due to excess mortality in the elesclomol arm specifically in patients with elevated baseline LDH — a biomarker of high glycolytic flux and oxidative stress. Crucially, the pre-specified normal-LDH subgroup showed no excess mortality. SDH-deficient tumors are constitutively pseudohypoxic but rely on reductive glutamine carboxylation (not aerobic glycolysis) for lipid synthesis; LDH-elevating aerobic glycolysis is not their dominant metabolic phenotype, which may mitigate the stratified risk signal. Current clinical context: NCT04710888 (Phase 1/2; elesclomol + CuSO₄ in advanced malignant pleural mesothelioma, FDX1-high tumors enriched) provides the most direct safety and dosing anchor for the cuproptosis mechanism in solid tumors. Key limitations: (1) No data test elesclomol in SDH-deficient models; the FASN→mtFAS→lipoic acid→cuproptosis chain in SDH-deficient cells is mechanistically grounded but experimentally untested. (2) FDX1 expression levels in SDH-deficient GIST, PPGL, and RCC are unknown; expression is highest in adrenocortical/steroidogenic tissue, making PPGL a biologically plausible priority. (3) The NCT00088010 Phase 3 safety signal requires careful attention to patient selection (LDH stratification) in any SDH-deficient trial design. Evidence_score 22 reflects a rigorous two-anchor mechanistic chain (SDH→FASN elevation, PMID 41520938; FDX1→cuproptosis, PMID 35588000) without direct SDH-deficient model validation.

Pathway Connections
De Novo Lipogenesis / FASN Dependency

SDH loss truncates the TCA cycle at the succinate → fumarate step, forcing cells to generate lipid precursors via reductive carboxylation of glutamine: glutamate → α-KG → isocitrate → citrate (reverse TCA via IDH1/IDH2), which is exported to the cytoplasm and cleaved by ATP-citrate lyase (ACLY) to yield acetyl-CoA. Fatty acid synthase (FASN) then converts acetyl-CoA and malonyl-CoA into palmitate and longer-chain fatty acids required for membrane biogenesis, lipid signalling, and mitochondrial lipid supply. Independently, FASN products are required for mitochondrial fatty acid synthesis (mtFAS), which produces the lipoic acid moiety needed by key mitochondrial enzyme complexes. A FASN-SDHB synthetic interaction was directly demonstrated using the FASN inhibitor G28UCM in SDHB-knockout cell lines: G28UCM impaired FASN activity and mitochondrial fatty acid synthesis more profoundly in SDHB-deficient cells than in WT controls, establishing selective synthetic lethality (Rodríguez-Flores et al., Pharmacol Res 2026, PMID 41520938).

Upstream event:

SDH loss → TCA cycle truncation at Complex II → reductive glutamine carboxylation as primary citrate-generation route → ACLY-mediated cytoplasmic acetyl-CoA production → upregulated FASN-mediated de novo fatty acid synthesis; concurrent dependence on FASN products for mitochondrial lipid supply and mtFAS

Downstream effects:

Reductive carboxylation of glutamine as primary lipid precursor route (replaces pyruvate-derived acetyl-CoA)Elevated FASN-mediated palmitate and long-chain fatty acid synthesisDependency on FASN products for mitochondrial membrane lipids and lipoic acid (via mtFAS)FASN inhibition (G28UCM) selectively impairs mitochondrial fatty acid synthesis and induces lethality in SDHB-deficient vs. WT cells (PMID 41520938)Dual cytoplasmic + mitochondrial lipid impairment under FASN inhibition exceeds the threshold tolerated by SDH-compromised cells
Cuproptosis via Lipoylation-mtFAS Vulnerability

SDH loss constitutively upregulates FASN-dependent lipid synthesis (Rodríguez-Flores et al. Cancer Res 2026, PMID 41520938); FASN supplies mitochondrial fatty acid synthesis (mtFAS) with octanoyl-ACP, the obligate precursor for lipoic acid biosynthesis. Elevated lipoic acid synthesis increases the lipoylated pool of TCA cycle proteins (DLAT, DLST). Copper ionophores (elesclomol) deliver Cu²⁺ intracellularly; FDX1 (ferredoxin-1) reduces Cu²⁺ → Cu⁺, which directly attacks lipoylated TCA proteins, triggering proteotoxic aggregation and cell death via cuproptosis (Tsvetkov et al. Science 2022, PMID 35588000). SDH-deficient cells, with constitutively elevated lipoylated TCA proteins via the FASN→mtFAS→lipoic acid chain, may be selectively vulnerable to copper ionophore-induced cuproptosis.

Upstream event:

SDH loss → succinate accumulation → FASN upregulation (PMID 41520938) → elevated FASN→mtFAS octanoyl-ACP flux → increased lipoic acid biosynthesis (LIPT1/LIPT2) → elevated lipoylated TCA proteins (DLAT, DLST) → heightened FDX1-mediated Cu⁺ attack sensitivity → cuproptosis

Downstream effects:

FDX1 reduces Cu²⁺ → Cu⁺; Cu⁺ directly attacks lipoylated DLAT and DLST, causing toxic aggregation and proteotoxic cell death (Tsvetkov et al. Science 2022, PMID 35588000)Elesclomol (STA-4783) is a copper ionophore that shuttles Cu²⁺ across the plasma membrane to mitochondrial FDX1, inducing cuproptosisFDX1 expression level is the primary determinant of cuproptosis sensitivity; lipoylated-DLAT aggregation is the proximal cytotoxic eventSDH-deficient cells exhibit FASN dependency (PMID 41520938); the FASN→mtFAS→lipoic acid flux elevation predicts an elevated lipoylated DLAT/DLST substrate pool that lowers the cuproptosis thresholdCuproptosis is mechanistically distinct from apoptosis, ferroptosis, and necroptosis — cells resistant to conventional cell death may retain cuproptosis sensitivityPhase I/II clinical context for elesclomol: NCT04710888 (advanced mesothelioma, FDX1-high tumors) establishes tolerability and dosing for the copper ionophore class
Molecular Targets

FDX1

Ferredoxin-1

direct

Mitochondrial ferredoxin-1 (FDX1; UniProt Q14213) is an iron-sulfur [2Fe-2S] electron-transfer protein located in the mitochondrial inner membrane space. FDX1 serves as the proximal electron donor for adrenal steroidogenesis (CYP11A1, CYP11B1) and is required for Fe-S cluster biogenesis. Tsvetkov et al. (Science 2022, PMID 35588000) identified FDX1 as the essential mediator of copper ionophore-induced cuproptosis: FDX1 reduces Cu²⁺ → Cu⁺, and Cu⁺ directly attacks lipoylated TCA cycle proteins — principally DLAT (E2 subunit of pyruvate dehydrogenase) and DLST (E2 subunit of α-ketoglutarate dehydrogenase) — triggering their toxic aggregation and proteotoxic cell death. FDX1 knockdown confers cuproptosis resistance; FDX1 overexpression sensitizes cells. Relevance to SDH-deficient tumors: SDH loss drives FASN upregulation (Rodríguez-Flores et al. Cancer Res 2026, PMID 41520938), and FASN supplies mtFAS with octanoyl-ACP, the obligate lipoic acid precursor. Elevated lipoic acid biosynthesis in SDH-deficient mitochondria increases the lipoylated DLAT/DLST pool available for FDX1-mediated Cu⁺ attack, mechanistically predicting reduced cuproptosis threshold relative to SDH-intact cells. PPGL may be particularly relevant given high intrinsic FDX1 expression in adrenocortical/chromaffin-origin tissue (adrenal steroidogenesis). FDX1 is a predictive biomarker of cuproptosis sensitivity rather than a conventional pharmacological target; copper ionophores (elesclomol) are the drug class that engages FDX1 catalytic activity to generate cytotoxic Cu⁺.
